CHRONIC DISEASE MANAGEMENT
We aim to monitor care and treatment for chronic disease with an appropriately trained nurse at least annually.
We aim to see all patients with asthma or chronic obstructive pulmonary disease at least once a year.
It is important that you arrange for a review when reminded. If treatment for respiratory diseases is not monitored annually lasting damage can be done to your airways.
HYPERTENSION / CORONARY HEART DISEASE CLINIC
We aim to see patients every six months. It is important to monitor blood pressure every six months, even if it has been stable. Uncontrolled blood pressure can increase the chances of someone having further heart problems or stroke.
Every year blood tests will be arranged to monitor any effects medications may be having, and to check other risk factors of heart disease/stroke such as cholesterol.
Every patient with diabetes should have an annual review. As part of this you will be asked to have some blood tests and provide a urine sample to screen for kidney disease. In addition to your annual eye screening, the nurse will test you blood pressure and foot sensation.
If there are any problems you may be asked to make a further appointment with the doctor.
Available each autumn, this vaccination is particularly recommended for patients aged 65 and over, all patients with diabetes, heart, chest or kidney disease and residents of nursing and rest homes.

Our nurses are able to advise on contraception and cervical screening. We also have a specialist Nurse, Carol Davies, who can fit and remove coils and implants.
We run general nursing clinics for wound care, removal of sutures, injections most days of the week.
Opportunistic screening, e.g. the measurement of blood pressure, comprises an important part of the normal consultation. We would prefer to prevent diseases before they start.
